• click on the green “Start Here” button
• click on the “Yes, I’m Ready” button
• provide all the required information on the “Your Information” page and continue forward
• provide all the required information on the “For Your Security” page and continue forward
• provide all the required information on the “Verify Your Account” page and continue forward
• provide all the required information on the “Additional Options” page and continue forward
• once you have provided all the required information you will receive a confirmation page and you may now need to click on the “Activate my card” link
